I served a S.21 which expires on 15 August, I’ve heard today that tenant’s child is in foster care due to alcohol addiction and tenant is having assessment by social services etc. The guarantor is also keen for the tenancy to end. Does the guarantor have legal authority to end the tenancy? If the tenant won’t leave the property, what happens?

No, the guarantor isn’t able to end the tenancy.
After the expiry of the notice, you would need to obtain a possession order through the courts.
